We provide for young people
- Fully furnished accommodation
- Ongoing assessments of young people’s needs
- Regular reviews of young people’s progress
- Monthly progress reports sent to local authorities
- Registration with GP, Dentist and Opticians
- Life skills programme
- Key work sessions (minimum 5 hours a week)
- Key worker is allocated within 2 hours of young person’s admission
- Key work sessions starts within 24 hours of admission
- Active engagement in education, training and/or employment
- Make contributions to the young person’s pathway plan
- Facilitate LAC Reviews and Professionals meetings at our accommodation
We provide specialist semi-independent accommodation and support services to young people aged 16 years and over. Our 24 hour accommodations cater for young people with a higher degree of vulnerabilities. This includes young people with emotional and behavioural difficulties and young people with very limited independent living skills.
These accommodations are staffed 24 hours a day with qualified transition key workers. Our transition key workers have experience of working with children and young people with challenging behaviour including young people with offending behaviour, mental health issues and young people who often go missing from placements.
We provide a minimum of 5 hours key work session every week and ensure that we make face to face contact with young people on a daily basis. When situation warrants, appropriate safe guarding protocol are triggered to minimise risks to young people.
We also have accommodation for young people who possess some independent living skills but are yet to develop all the skills and competence to live independently. These accommodations are staffed from 9pm to 9am.
All young people will have contact details of their key worker and Acorn management team in case of any emergencies.
Additional support can be provided to young people at the request of the placing authorities.
We are not just about educating young people to develop independent living skills. Our holistic interventions are designed to transform the mindset of young people so that they can achieve great things in life! We achieve this by enabling and empowering young people through understanding, compassionate care and with carefully thought out programme of support. We are persistent in our motivations to move young people from dependency to independence.
At Acorn we provide an environment where young people have all the opportunities to demonstrate their independent living skills.
